EUROPEANIZATION OF EYELIDS OR PLASTY OF ASIAN EYELIDS

The special structure of the eyelids, i.e. The narrow shape of the eyes, the smoothness or absence of the fold of the upper eyelid in people of Asian nationality is often combined with the “Mongolian” fold, which is called the “epicanthus”. With significant development, the “Mongolian” fold covers the inner corner of the eye, the lacrimal caruncle and the beginning of the lacrimal ducts.

Europeanization of eyelids or plastic surgery of Asian eyelids is a type of blepharoplasty, but it has its own characteristics and subtleties. The essence of Europeanizing blepharoplasty is the formation of a clear, beautiful fold of the upper eyelid, the removal of excess fatty hernias, which makes the eyes more open and expressive. Sangapuri blepharoplasty is a purely aesthetic operation and is performed only at the request of the patient.

First of all, we need to figure out: what is the oriental eyelid (Eastern, Asian)? What is characteristic of the Asian century and of oriental eyes in general? This:

Lack of folds on the upper eyelid;

Epicanthus: fold of skin covering the inner corner of the eye;

Asian slant;

Almond shape of the eye.

ABOUT The absence of a fold is due to the fact that the skin is not fixed to the muscle that lifts the eyelid. Therefore, when the eyes open, instead of the formation of a fold, the reverse process occurs, the skin seems to lie in a fold. Not inside the eye, but outside. Another reason is excess subcutaneous fat (not to be confused with orbital fat, i.e. with “hernias”). During surgery, excess subcutaneous fat is removed and the skin is sutured to the levator palpebral muscle. This is how the missing fold of the upper eyelid is formed.

P In almost all people, the distance between the eyes is equal to the width of the eye. Due to the epicanthal ligament, those with an oriental eyelid structure have their eyes slightly closed at the inner corner - this visually makes the eye smaller and at the same time makes the bridge of the nose wider. It turns out to be such an injustice: anatomically the eyes are at the required width, the eye itself is not small, but everything looks quite the opposite - a wide bridge of the nose and narrow eyes. When we remove the epicanthus fold, we eliminate the optical illusion. The eye will become its true size, and the bridge of the nose will be narrower, and you will get almost ideal proportions. What is important is that the shape of the eyes, in the end, will remain its own and will not change.

E two more concepts characteristic of the Asian century: almond shape and Asian obliquity. Epicanthus can be extremely pronounced. It turns out that the distance to the pupil from the inner corner of the eye is half as long as from the outer corner. Remember any portrait drawn: no matter where you stand, it seems that the portrait is looking at you. Why? Because the pupil in the portrait is in the center of the eye. And in the case of pronounced epicanthus, the pupil seems to be shifted to the inner corner. This happens if you squint your eyes. This is Asian slanting. Those. The eye actually looks straight, but it seems to look at the finger in front of it. And when we remove the epicanthus, we solve this problem.
The almond shape of the eye is a mystery. One must assume that the eye resembles an almond, but almonds only resemble the Asian eye in profile. Therefore, by amygdala I mean a narrow palpebral fissure. This problem is solved along with the formation of a fold.


D O - pronounced epicanthus, the distance from the center of the pupil to the outer edge of the eye is almost twice the distance to the inner corner. Wide bridge of nose.

P OSLE - two weeks after removal of the epicanthus and formation of the fold. The distance from the center of the pupil to the outer corner is almost equal to the distance to the inner corner. The bridge of the nose looks much narrower. The true size of the eye is revealed.

1. The surgical field is being processed. I prefer three times treatment with 70% alcohol and one time treatment with 96% alcohol over various synthetic products. The less various types of synthetics come into contact with the body, the better.

2. A very important stage is marking. For eyelids, this is more than half of the operation, since every millimeter on the eyelids plays a role. For example, the fold should be at a distance of 5-8 mm above the eyelashes, only then will it be beautiful.

3. The jaws of the instrument indicate the distance required to move tissue to remove the epicanthus. The question is again measured in mm.

4. The outer corner of the patient's incision was made with a slight s-shaped tail due to the pronounced excess of excess skin in order to reduce the length of the incision.

5. Local anesthesia with a 1% solution of lidocaine and adrenaline is the ideal combination for this operation. Of course, an insulin syringe with a thin needle, to reduce discomfort from communicating with the doctor.

6. It is extremely difficult to make a cut with a scalpel along a thin line on the skin of the eyelids. After anesthesia, the thin line becomes up to 2-3 mm wide and it is difficult to understand which edge of this line to make the incision on: the outer, the inner, or in the middle... (True, this takes a couple of hundred operations)

7. "Doctor!!! I see the light!!!" - this is exactly what patients say (they often scream in fear if not warned) at the moment when excess skin is removed and the bright light of the operating lamp literally “hits” the eyes.

8. All that was acquired through backbreaking labor is three centimeters of skin.

9. Electrocoagulation of blood vessels to stop bleeding.

10. Epicanthus plastic surgery. It is necessary to move the inner corner of the eye (excess skin has already been removed) and extend the incision to the lower eyelid to avoid recurrence of the epicanthus. The epicanthus appears due to a lack of skin on the lower eyelid, that is, it is not the skin of the upper eyelid that stretches down, but the skin of the lower eyelid, on the contrary, stretches upward and pulls the epicanthus. Therefore, epicanthus plastic surgery is performed in such a way as to relieve tension on the lower eyelid.

11. Fitting. It happens that the surgeon does not get it right the first time, which means that it is necessary to further prepare, trim, etc. Therefore, just as an artist adds the finishing touches to a picture, the surgeon tries it on very often during the operation. It can be seen that there is enough tissue to open the inner corner of the eye well.

12. A small tunnel is created for immersing the knot from the seam into it, which will hold the fold.

13. A suture is placed to secure the fold along the entire length of the incision (with Prolene 6-0 thread). The thread is non-absorbable and will hold the fold. In China, such stitching is done directly through the skin with a silk thread, then the thread is removed, and the scar tissue that forms around the thread acts as a holding suture. This method is applicable for true Asian eyelids in young patients, where there is no excess skin and epicanthus is not performed. The advantage is the absence of cuts. The disadvantage is the low predictability of the result.

14. The first seam of the fold. The correctness of the application is checked: we ask you to open your eyes and if the seam stretches behind the eyelid, it means that it is caught at the required depth. You can apply sutures by cutting the muscle along the entire perimeter of the wound and visually fix the edge of the skin to the cartilaginous plate or to the aponeurosis of the muscle that lifts the eyelid. This method is more reliable, but actually more traumatic. This means rehabilitation is much more difficult.

15. The fold is formed on one side. The difference with the unoperated eye is clearly visible.

16. The operation is completed. Two gauze bandages will be applied on top, which the patient will remove at home herself the next day.

The Singapore operation is intended to correct the Asian shape of the eyes in order to give it a Caucasian appearance.

The first Asian eyelid surgery was performed at the end of the 18th century. It is generally accepted that the birthplace of this word is the country with the same name. But that's not true. It’s just that “Singapore” is a word that is more convenient to pronounce and has already become established in speech for Russian speakers. In fact, the spelling looks like this - “sangapuri”. This word is of Korean origin and means “eyes without epicanthus” (the epicanthus is a vertical crescent-shaped fold of skin covering the inner corner of the palpebral fissure; a continuation of the fold of the upper eyelid).

Singaporei. How is the operation performed?

The operation is usually performed under general anesthesia (but sometimes under local anesthesia) and takes from 1 to 2 hours. There are two methods of performing Singapore - suture (without incisions) and open (with incisions). The plastic surgeon individually selects the most suitable method of Asian blepharoplasty for each patient, depending on the degree of complexity.

First method. Suture

This technique is performed if the patient has slight drooping and thin skin of the upper eyelids. The main advantage of this method is the absence of a postoperative scar. In addition, the suture method allows you to redo the result if it turns out to be unsatisfactory.

At the beginning of the operation, the surgeon marks a line for the future skin fold of the upper eyelid. Next, tiny punctures (1 mm) are made in the skin of the eyelid. Then the doctor stitches all the tissues with an elastic surgical thread between the skin and the aponeurosis of the muscle that raises the upper eyelid (levator muscle). This is how the “European” fold is formed.

Second method. Open

Used in the vast majority of cases. Unlike the suture method, with the open Singapore method there will be an invisible postoperative scar.

The surgeon cuts the upper eyelid along the ciliary edge. The higher from the lash line it is created, the higher the fold will be, and the larger the eyes will appear. The doctor then excises the skin and subcutaneous structures by advancing the scalpel toward the orbicularis oculi muscle. The doctor cuts off a narrow strip from it. The next stage is dissection of the orbital septum, behind which the intraorbital fat is hidden.

When forming a deep “European” fold, the surgeon excises almost all of the orbital fat. In the case of double and small folds, little or no fat is removed.

The surgeon then removes part of the levator muscle. This is necessary for subsequent rapid tissue fusion, without noticeable tension.

At the end of the operation, the surgeon stitches the incised skin along with the levator aponeurosis of the upper eyelid. In addition to the fixing sutures placed in certain places of the eyelid, one continuous suture is applied along the entire incision line, and a disinfecting bandage is placed on top of it.

Singaporei. Rehabilitation

The bandage is removed the next day. On days 5-7, the sutures are removed, after which significant scars remain, which are hidden in the formed folds of the upper eyelids. Soft tissue swelling and bruising will disappear within two weeks. To relieve postoperative symptoms, painkillers, antiseptics, and decongestants can be prescribed. During the month after surgery, you should adhere to the following recommendations:

  • exclude physical activity;
  • limit the use of cosmetics;
  • refuse contact lenses;
  • when outdoors, wear sunglasses without taking them off;
  • if swelling appears, do not use ointments, but apply ice;
  • refuse reading, which causes severe eye strain;
  • completely eliminate alcohol and smoking;
  • Avoid any eye irritants.
