The active ingredient of Preductal is trimetazidine dihydrochloride. Has anti-ischemic and antianginal effects. Preductal is a modified-release trimetazidine tablet (extended release). Preductal - short-acting tablets. Trimetazidine helps stabilize energy metabolism in cells in the presence of ischemia or hypoxia by preventing a decrease in the level of adenosine triphosphate inside cells. Thanks to this, normalization of the functioning of ion pumps and sodium-potassium transmembrane flow is observed while maintaining cell homeostasis. The active ingredient Preductal partially inhibits the oxidation of fatty acids due to the suppression of 3-CAT - long-chain 3-ketoacyl CoA thiolase. This normalizes energy metabolism in cardiac cells, causes an increase in glucose oxidation and improves glucose oxidation associated with glycolysis, which protects myocardiocytes from ischemic damage.

Preductal also enhances phospholipid metabolism and the incorporation of phospholipids into cell membranes, due to which stabilization of membranes is observed under conditions of ischemia or hypoxia. In hypoxia or ischemia, the most optimal energy metabolism is glucose oxidation; trimetazidine switches metabolism from fatty acid oxidation to a more favorable pathway - glucose oxidation, which explains the antianginal effect of Preductal.

Thanks to the dosage form with modified release of trimetazidine, Preductal has an optimal pharmacokinetic profile. During the day, the plasma concentration of the active substance Preductal remains at least 75% of the maximum for 11 hours. Compared to short-acting Preductal, the minimum effective concentration in blood plasma is 31% higher. This ensures stable effectiveness of the drug over 24 hours, including the early hours of the day, when the likelihood of cardiovascular complications is highest. Thus, prolonged Preductal provides a round-the-clock reliable cardioprotective and anti-ischemic effect.

After oral administration, the equilibrium concentration of trimetazidine in the blood plasma is achieved no later than 60 hours after the start of use. The pharmacokinetic characteristics of modified-release tablets are not affected by food intake. The volume of distribution indicator is 4.8 l/kg. Only a small part of the active substance is combined with plasma proteins - approximately 16%. It is eliminated predominantly in unchanged form by the kidneys. The half-life in young healthy volunteers is 7 hours, in people over 65 years of age – 12 hours. No dosage adjustment is required in the elderly.

Application in cardiology
In clinical studies with a placebo-controlled, double-blind design, trimetazidine was shown to increase the threshold for myocardial ischemic injury and ischemia, as assessed by the following criteria:

  • change in exercise tolerance;
  • studying the severity and time of appearance of ST segment elevation on the ECG;
  • degree of increase in coronary reserve;
  • reducing the need to prescribe nitroglycerin in usual doses;
  • reducing the severity and frequency of angina attacks.

In clinical studies, Preductal was prescribed both as part of complex treatment and as monotherapy; both in the treatment of chronic and acute conditions. It has been established that during hypoxia and ischemia, trimetazidine improves energy metabolism in neurosensory organs and the myocardium, prevents changes in ionic transmembrane flows that are associated with ischemia, reduces the severity of acidosis within cells, reduces the ability of polymorphonuclear neutrophil granulocytes to infiltrate and migrate into reperfused and ischemic myocardium . The drug also reduces the severity of ischemic-associated disorders and significantly reduces the size of myocardial infarctions that are caused experimentally. Does not change hemodynamic parameters.
In controlled clinical trials in patients with angina pectoris, trimetazidine has been shown to delay the onset of ischemia by increasing coronary reserve from approximately the 15th day of therapy under conditions of increased stress; significantly reduces the incidence of angina attacks; limits sharp fluctuations in blood pressure without clinically significant changes in heart rate; helps reduce the dosage of nitroglycerin. Clinical studies of trimetazidine for 2 months under conditions of dosed physical activity revealed that when used at a dosage of 35 mg in combination with atenolol (50 mg/day), there is an increase in the time interval from ischemia to visible changes in the ST segment (by 1 mm) compared with placebo already 12 hours after administration.

Application in otolaryngological practice
In clinical studies of a placebo-controlled, double-blind design, Preductal was shown to effectively reduce the frequency, duration and severity of dizziness attacks due to its anti-ischemic effect and protection of vestibular neurons from excitotoxic amino acids that have a toxic effect. The drug reduces the frequency and intensity of tinnitus and reduces the likelihood of its recurrence. Expands the perceived sound range in case of perceptual deafness.

Application in ophthalmological practice
When taking the drug, an increase in the b-wave amplitude is recorded on the electroretinogram, which is an indicator of ischemia of the organ of vision. When taking Preductal, the prognosis for restoration of the functional activity of the retina improves, the field of vision increases and visual acuity increases due to the improvement in the morphological structure of the retina and macula after taking trimetazidine (including cases of degenerative age-related changes).

Trimetazidine (the active ingredient of the drug Preductal) slows down the oxidation of fatty acids due to the selective inhibition of long-chain 3-ketoacetyl-CoA thiolase, which leads to increased glucose oxidation and restoration of the coupling between glycolysis and oxidative decarboxylation and protects the myocardium from ischemia. The switch from fatty acid oxidation to glucose oxidation underlies the antianginal effect of trimetazidine.

It has been experimentally confirmed that Preductal has the following properties:

  • supports energy metabolism of the heart and neurosensory organs during ischemia;
  • reduces the amount of intracellular acidosis and the degree of changes in the transmembrane ion flow that occurs during ischemia;
  • reduces the level of migration and infiltration of polynuclear neutrophils in ischemic and reperfusion tissues of the heart;
  • reduces the size of myocardial damage;
  • does not have a direct effect on hemodynamic parameters.

In patients suffering from angina, Preductal:

  • increases coronary reserve, thereby slowing down the development of ischemia caused by physical activity, starting from the 15th day of therapy;
  • limits sharp fluctuations in blood pressure caused by physical activity without any significant changes in heart rate;
  • significantly reduces the frequency of angina attacks and the need for short-acting nitroglycerin;
  • improves contractile function of the left ventricle in patients with ischemic dysfunction.

In clinical studies lasting 2 months, it was shown that the addition of Preductal MB at a dose of 35 mg to therapy with atenolol at a dose of 50 mg after 12 hours led to a significant delay in the onset of ischemic depression of the ST segment during stress tests.

Pharmacokinetics

After taking the drug orally, Preductal is quickly absorbed. Food intake does not affect the pharmacokinetic properties of the drug. It is excreted from the body mainly by the kidneys unchanged.

Indications

  • cardiology: long-term therapy of coronary artery disease, prevention of attacks of stable angina (monotherapy or as part of combination therapy);
  • ENT diseases: treatment of cochleo-vestibular disorders of ischemic nature, such as dizziness, tinnitus, hearing impairment;
  • ophthalmology: chorioretinal disorders with an ischemic component.

Release forms

Film-coated tablets 20 mg and 35 mg (Preductal MR).

Instructions for use and dosage regimen

Preductal MB is prescribed 1 tablet 2 times a day during meals in the morning and evening. The duration of therapy is determined individually.

I take the tablets whole orally, without chewing and with water.

Side effect

  • stomach ache;
  • diarrhea;
  • dyspepsia;
  • nausea, vomiting;
  • dizziness;
  • headache;
  • extrapyramidal symptoms (tremor, rigidity, akinesia), reversible after discontinuation of the drug;
  • feeling of heartbeat;

    extrasystole;

    tachycardia;

    pronounced decrease in blood pressure;

    orthostatic hypotension, which may be accompanied by general weakness, dizziness or loss of balance, especially when taking antihypertensive drugs simultaneously;

  • rushes of blood to the skin of the face;
  • skin rash;
  • hives;
  • asthenia.

Contraindications

  • severe renal failure (creatinine clearance less than 15 ml/min);
  • hypersensitivity to the components of the drug;
  • children and adolescents under 18 years of age.

Use during pregnancy and breastfeeding

Experimental studies have not established the teratogenic effect of trimetazidine. However, due to the lack of clinical data, the drug is not recommended for use during pregnancy.

It is not known whether trimetazidine is excreted in breast milk. Therefore, if it is necessary to prescribe the drug during lactation, breastfeeding should be stopped.

Use in children

Due to the lack of sufficient clinical data, it is not recommended to prescribe the drug to children and adolescents under the age of 18 years.

Use in elderly patients

Preductal MB should be prescribed with caution to elderly patients over 75 years of age, in whom increased exposure may occur.

special instructions

Preductal MV is not intended for the relief of angina attacks and is not indicated for the initial course of treatment of unstable angina or myocardial infarction in the prehospital stage or in the first days of hospitalization.

If an attack of angina occurs, treatment (drug therapy or revascularization procedure) should be reviewed and adapted.

Preductal MB can cause or worsen symptoms of parkinsonism (tremor, akinesia, increased tone), so patients should be regularly monitored, especially the elderly. In doubtful cases, patients should be referred to a neurologist for appropriate examination.

If movement disorders appear, such as symptoms of parkinsonism, restless legs syndrome, tremor, instability in the Romberg position and unsteadiness of gait, Preductal MB should be permanently discontinued.

Such cases are rare and symptoms usually resolve after discontinuation of therapy: in most patients, within 4 months after discontinuation of the drug. If symptoms of parkinsonism persist more than 4 months after discontinuation of the drug, you should consult a neurologist.

There may be cases of falls associated with instability in the Romberg position and “wobbly” gait or a pronounced decrease in blood pressure, especially in patients taking antihypertensive drugs.

Preductal MB should be prescribed with caution to patients in whom increased exposure may occur:

  • with moderate and severe renal failure;
  • in elderly patients over 75 years of age.

Impact on the ability to drive vehicles and operate machinery

Clinical studies did not reveal the effect of the drug Preductal MB on hemodynamic parameters, however, during the period of post-registration use, cases of dizziness and drowsiness were observed, which may affect the ability to drive vehicles and perform work that requires an increased speed of physical and mental reactions.
