Receiving documents from applicants.

4.1. Acceptance of documents for the first year is carried out from June 20, 2016. until August 10, 2016 If there are free places for admission to the College, acceptance of documents may last until November 25, 2016.

Admission to the College for the first year is carried out on the personal application of citizens.

4.2. When submitting an application (in Russian) for admission to the Technical School, the applicant presents:

Identity and citizenship document (passport or photocopy);

At your discretion, the original or photocopy of a state-issued education document:

a) certificate of basic general or secondary general education (mandatory provision);

b) diploma of secondary vocational education (with an appendix);

c) diploma of higher professional education (with an appendix);

Insurance policy;

Insurance certificate of state pension insurance (copy)

An extract from the work record book, certified by the administration (original at the time of enrollment), for persons with work experience;

Persons who have completed military service by conscription or have been discharged from military service provide a military ID upon admission to the Technical School;

The original or copy of a medical certificate containing information about a medical examination in the manner established when concluding an employment contract or service contract for the relevant profession or specialty, approved by the Decree of the Government of the Russian Federation dated August 14, 2013. If the applicant fails to provide or has an invalid medical certificate, or if it does not contain, in whole or in part, information about a medical examination that meets the requirements, the College informs the applicant about the time and place of the medical examination, posting this data on the information stand and on the official website of the College.

4.2.1. The diseases listed in the order of the Ministry of Health and Social Development of the Russian Federation dated April 12, 2011 No. 302n section 4 are a contraindication for admission to the College specifically:

Congenital malformations, deformations, chromosomal abnormalities with persistent severe dysfunction of organs and systems;

Consequences of damage to the central and peripheral nervous system, internal organs, musculoskeletal system and connective tissue from exposure to external factors (trauma, radiation, thermal, chemical and other effects, etc.) with the development of irreversible changes that caused dysfunction of organs and systems pronounced degree;

Diseases of the central nervous system of various etiologies with severe motor and sensory impairments, coordination and static disorders, cognitive and mnestic-intellectual impairments;

Narcolepsy and cataplexy;

Diseases accompanied by disorders of consciousness: epilepsy and epileptic syndromes of various etiologies, syncope syndromes of various etiologies, etc.;

Mental illnesses with severe, persistent or often exacerbating painful manifestations and conditions equivalent to them, subject to mandatory dynamic observation in psychoneurological dispensaries;

Alcoholism, substance abuse, drug addiction;

Diseases of the endocrine system of a progressive course with signs of damage to other organs and systems and disruption of their function of 3-4 degrees;

Malignant neoplasms of any location;

Diseases of the blood and hematopoietic organs with a progressive and recurrent course (hemoblastosis, severe forms of hemolytic and aplastic anemia, hemorrhagic diathesis);

Hypertension stage III, degree 3, risk IV;

Chronic heart and pericardial diseases with circulatory failure FC III, NC 2 or more degrees;

Cardiac ischemia:

1. angina pectoris FC III - IV;

2. with conduction disturbances (sinoauricular block of the third degree, weakness of the sinus node);

3. paroxysmal rhythm disturbances with potentially malignant ventricular arrhythmias and hemodynamic disturbances;

4. post-infarction cardiosclerosis, cardiac aneurysm;

5. aneurysms and dissections of any parts of the aorta and arteries;

6. obliterating atherosclerosis of the aorta with obliteration of the visceral arteries and dysfunction of organs;

Obliterating atherosclerosis of the vessels of the extremities, thromboangiitis, aortoarteritis with signs of decompensation of the blood supply to the extremity (limbs);

Varicose and postthrombophlebitic disease of the lower extremities with symptoms of chronic venous insufficiency of grade 3 and higher;

Lymphangitis and other disorders of lymphatic drainage 3 - 4 degrees;

Rheumatism: active phase, frequent relapses with damage to the heart and other organs and systems and chronic heart failure of 2 - 3 degrees;

Diseases of the bronchopulmonary system with symptoms of respiratory failure or pulmonary-heart failure of 2 - 3 degrees;

Active forms of tuberculosis of any localization;

Complicated course of gastric ulcer, duodenal ulcer with chronic often (3 times or more per calendar year) recurrent course and development of complications;

Chronic hepatitis, decompensated liver cirrhosis and other liver diseases with signs of grade 2 - 3 liver failure and portal hypertension;

Chronic kidney and urinary tract diseases with symptoms of chronic renal failure of 2 - 3 degrees;

Nonspecific ulcerative colitis and severe Crohn's disease;

Diffuse diseases of connective tissue with dysfunction of organs and systems of 3 - 4 degrees, systemic vasculitis;

Chronic diseases of the peripheral nervous system and neuromuscular diseases with significant impairment of function;

Chronic diseases of the musculoskeletal system with dysfunction of 2 - 3 degrees;

Chronic skin diseases:

1. chronic widespread, often recurrent (at least 4 times a year) eczema;

2. psoriasis universal, widespread, arthropathic, pustular, psoriatic erythroderma;

3. pemphigus vulgaris;

4. chronic irreversible widespread ichthyosis;

5. chronic progressive atopic dermatitis;

Pregnancy and lactation;

Recurrent miscarriage and a history of fetal anomalies in women of childbearing age;

Glaucoma of any stage with an unstabilized course.

If an applicant has medical contraindications, the College ensures that he is informed about the consequences associated with these contraindications during his studies at the College and subsequent professional activities.

When submitting an application:

When submitting an application, disabled people or persons with limited health capabilities submit, at their discretion, an original or a photocopy of one of the following documents:

a) conclusion of the psychological, medical and pedagogical commission;

b) a certificate of disability issued by the federal institution of medical and social examination;

Children with disabilities of groups 1 and 2 present, at their discretion, an original or a photocopy of a certificate of disability and a conclusion about the absence of contraindications for studying at the Technical School, issued by the Federal Institute of Medical and Social Expertise, a rehabilitation program card;

If it is necessary to create special conditions during entrance examinations - disabled people and persons with limited health capabilities, additionally - a document confirming the disability or limitation of health capabilities that require the creation of the specified conditions.

4.2.2. Orphans and children left without parental care in accordance with Federal Law No. 159-FZ of December 21, 1996 “On additional guarantees for social support for orphans and children left without parental care” additionally attach the following documents:

A copy of the act (resolution, termination, order) establishing guardianship (trusteeship) of the child;

A copy of the child’s birth certificate, and if information about the child’s father is included in the birth certificate based on an application from the child’s mother, a certificate from the civil registry office on the basis for including information about the father in the child’s birth certificate;

A certificate from the place of residence (stay) of the guardian (trustee) about his joint residence with the child under his ward or an act of actual joint residence;

Copies of documents confirming the fact of lack of parental (sole parent) custody of the child;

a) death certificate of parents;

b) a court decision to deprive parents of parental rights (on restrictions on parental rights), declaring parents incompetent (partially capable), missing or declaring parents dead;

c) a document confirming that the parents are in custody or serving a sentence of imprisonment;

d) a document confirming the discovery of a found (abandoned) child, issued in accordance with the procedure established by the legislation of the Russian Federation;

e) an application from parents for consent to the adoption of a child raised (stayed) in an institution for orphans and children left without parental care, as well as in other educational, training, medical institutions or other similar institutions, drawn up in the prescribed manner ;

f) a copy of a certificate issued by the federal state institution of medical and social examination confirming the fact that the parents (only parent) have been diagnosed with disability of group 1 or 2;

g) a court decision to establish the fact of the absence of parental care over the child (including in connection with the illness of the parents) or to exclude information about the parent (parents) from the birth record;

h) a certificate from the internal affairs authorities stating that the location of the wanted parents has not been established;

i) a copy of the act of abandonment of the child by the mother, who did not present an identity document, in the medical organization in which the birth took place or to which the mother applied after childbirth;

j) birth certificate of a child, in the lines “Father” and “Mother” there are dashes.

4.3. When submitting an application (in Russian) for admission to the College, foreign citizens, stateless persons, including compatriots living abroad, present the following documents:

A copy of the identity document of the applicant, or the identity document of a foreign citizen in the Russian Federation, in accordance with Article 10 of the Federal Law of July 25, 2002. No. 115-FZ "On the legal status of foreign citizens in the Russian Federation";

The original of a state-issued document on education (or its duly certified photocopy), or the original document of a foreign state on levels of education and (or) qualifications, recognized in the Russian Federation at the level of a state-issued document on education (or its duly certified copy) , as well as in the case provided for by the legislation of the Russian Federation, a copy of the certificate of recognition of this document;

A duly certified translation into Russian of a foreign state’s document on education and (or qualifications) and its annex (if the latter is provided for by the legislation of the state in which such a document on education was issued);

Copies of documents or other evidence confirming that a compatriot living abroad belongs to the groups provided for in Article 4 of the Federal Law of May 24, 1999 No. 99-F3 “On the state policy of the Russian Federation regarding compatriots abroad”;

6 photographs measuring 3x4cm (photos without headgear).

All translations into Russian must be made in the name and surname indicated in the identity document of a foreign citizen in the Russian Federation.

4.4. Applicants must provide the following required information in their application:

last name, first name, patronymic (last - if available);

Date and place of birth;

details of his identity document, when and by whom it was issued;

location

information about the previous level of education and the document on education and (or) the document on education and the qualifications confirming it;

the specialty for which he plans to enroll at the Technical College, indicating the conditions of study and the form of education (within the target numbers for admission, places under contracts for the provision of paid educational services with payment of the cost of training by individuals and (or) legal entities);

(C)- (C) the need for a hostel;

(C)- (C) the need to create special conditions for the applicant during admissions tests due to his disability or limited health capabilities.

The application also records the fact of familiarization (including through public information systems) with the Charter, copies of the license to carry out educational activities, certificates of state accreditation and appendices thereto. The fact of familiarization is certified by the personal signature of the applicant.

The applicant’s signature also confirms the following:

receiving secondary vocational education for the first time;

familiarization (including through public information systems) with the date of provision of the original document on education and (or) document on education and qualifications - before 12.00 08/16/2016 G;

consent to the processing of your personal data in the manner established by the Federal Law of July 27, 2006. No. 152-F3 “On personal data”.

If an applicant submits an application that does not contain all the information provided for in this paragraph and (or) information that does not correspond to reality, the College returns the documents to the applicant.

4.5. Applicants have the right to send an application for admission, as well as the necessary documents, through public postal operators (hereinafter - by mail), as well as in electronic form (if such a possibility is provided in the educational organization) in accordance with the Federal Law of April 6, 2011. No. 63-FZ “On Electronic Signature”, Federal Law of July 27, 2006. No. 149-FZ “On information, information technologies and information protection”, Federal Law of July 7, 2003. No. 126-FZ “On Communications”. When sending a document by mail, the applicant shall attach to the application for admission a photocopy of documents proving his identity and citizenship, a document on education and (or) a document on education and qualifications, as well as other documents provided for by these Rules.

Documents sent by mail are accepted upon their receipt by the College no later than the deadlines established by paragraph 4.1 of these Rules to complete the acceptance of documents.

When submitting the original documents in person, applicants are allowed to have their photocopies certified by the Technical School.

4.6. It is not allowed to charge fees for those arriving when submitting the documents specified in paragraph 4.2 of these Rules.

4.7. Reception of documents is recorded in a journal of a prescribed form, with mandatory page numbering, laced and sealed with the seal of the “Technical School”.

On the day the documents are accepted, entries in the journal are closed with a final line signed by the chairman and executive secretary of the selection committee indicating the number of accepted applications.

4.8. A personal file is opened for each applicant, in which all submitted documents and materials for passing entrance examinations are stored.

Note: The personal file number corresponds to the number in the applicant registration register.

4.9. Registration logs and personal files of applicants are stored as strictly accountable documents under the financial responsibility of the secretary of the admissions committee for six months from the date of acceptance of documents.

4.10. When submitting documents in person, the applicant is given a document acceptance receipt.

4.11. Upon written application, applicants have the right to pick up the original document on education and (or) document on education and qualifications, and other documents submitted by the applicant. Documents must be returned to the College within the next working day after submitting the application.

4.12. Applicants who have submitted deliberately false documents to the admissions committee are liable in accordance with the legislation of the Russian Federation.

Nursing is a specialty of secondary vocational education

Qualification - "Nurse"/"Medical Brother".Basic training. Full-time training on a budgetary and paid basis, duration of study is 3 years 10 months, on the basis of basic general education (9 classes).

A nurse is prepared to work in urban and rural children's and general medical institutions as specialists in caring for patients and performing medical procedures and at the same time to manage the work of ward nurses. The nurse should know: The main symptoms of various diseases, general and differentiated patient care. Treatment, indications and contraindications for the use and dosage of medications. Methods of dispensary services for the population, methods of sanitary educational work among patients, the population and training them in individual preventive measures. Methods of various preventive vaccinations. The nurse must be able to: observe and assess the condition of a patient of any age. Provide oral, nasal and skin care. Perform subcutaneous, intramuscular and intravenous injections. Perform gastric intubation and lavage. Measure blood pressure. Apply hemostatic tourniquets. Perform artificial respiration. Provide first aid for injuries, poisoning, frostbite, burns. Catheterize the bladder with a soft rubber catheter and rinse the bladder. Give various types of enemas. Conduct functional tests. Apply various types of bandages. Carry out and check the preparation of the patient for anesthesia, surgery, conduct inhalation anesthesia under the supervision of a doctor. Carry out sanitary and preventive work among patients and the population.

In 2019, the College announces enrollment in specialty 02/34/01 “Nursing”.

It is planned to recruit 50 people for training at the expense of budgetary allocations of the Chelyabinsk region.

It is planned to recruit 25 people for training under contracts for the provision of paid educational services (paid training).

Documents are accepted from 06/20/2019 to 08/09/2019.

If there are free places at the College, the acceptance of documents is extended until December 1, 2019.

Admission to the College is carried out on the personal application of citizens.

The procedure for establishing the College's target numbers for admitting citizens at the expense of the budget of the Chelyabinsk region is determined by the founder - the Ministry of Health of the Chelyabinsk region.

1.5. The conditions for admission to study at the Technical School must guarantee respect for the right to education and enrollment from among applicants who have the appropriate level of education, the most capable and prepared to master the educational program of the appropriate level and appropriate focus.

1.6. The technical school has the right to carry out, in accordance with the legislation of the Russian Federation in the field of education, the admission of citizens in excess of the established budget places for training on the basis of contracts with payment of the cost of training.

1.7. The technical school independently determines the procedure for organizing the admission of citizens for training under contracts with payment of the cost of training.

1.8. The technical school has the right to carry out targeted admissions of students within the limits of admission targets financed from the funds of the founder in accordance with agreements concluded with state authorities and local governments in order to assist them in training specialists of the relevant profile.

1.9. The College carries out the transfer, processing and provision of personal data received in connection with the admission of citizens in accordance with the requirements of the legislation of the Russian Federation in the field of personal data without obtaining the consent of these persons to process their personal data.

2. Organization of admission of citizens to the Technical School.

2.1. The organization of admission of citizens for training in mastering educational programs is carried out by the admissions committee of the Technical College (hereinafter referred to as the admissions committee).

The chairman of the admissions committee is the director of the Technical School.

2.2. The composition, powers and procedure for the activities of the selection committee are regulated by its regulations, approved by the director of the Technical School.

2.3. The work of the admissions committee and paperwork, as well as the personal reception of applicants and their parents (legal representatives), are organized by the responsible secretary of the admissions committee, who is appointed by the director of the Technical School.

2.4. Upon admission to the College, the rights of citizens in the field of education established by the legislation of the Russian Federation are ensured, the work of the admissions committee is transparent and open, and the assessment of applicants’ abilities and aptitudes is objective.


2.5. In order to confirm the accuracy of the documents submitted by applicants, the admissions committee has the right to contact the relevant state (municipal) bodies and organizations.

2.6. The technical school enters into the federal information system the information necessary for information support for the admission of citizens to educational organizations of secondary vocational education.

3. Organization of informing applicants.

3.1. The technical school announces the admission of citizens for training in educational programs in accordance with the license to carry out educational activities, specifically:

060101 “General Medicine”, qualification – paramedic, advanced level, full-time study, duration of study – 3 years 10 months on the basis of secondary general education (at the expense of the corresponding budget and under contracts with payment of the cost of training);

060203 “Dentistry-orthopedic”, qualification - dental technician, basic level, full-time, duration of study - 2 years 10 months on the basis of secondary general education, secondary vocational or higher education (under contracts with payment of tuition fees).

3.2. For the purpose of accessibility of information and familiarization of the applicant and his parents (legal representatives) with:

A list of specialties for which the College announces recruitment;

Recruitment plan for the chosen specialty (admission target numbers);

The number of places for admission in each specialty with payment of the cost of training on a contractual basis (paid training);

The number of applications submitted for training in the chosen specialty;

Charter of the College;

License for the right to conduct educational activities in the specialties being taught (series A No. 0 registration number 6969 dated January 1, 2001, Ministry of Education and Science of the Chelyabinsk Region);

State accreditation for trained specialties (No. 000 dated January 1, 2001, Ministry of Education and Science of the Chelyabinsk Region);

5. Entrance tests.

5.1. When entering the specialty “Orthopedic Dentistry” on a contractual basis, entrance examinations are carried out in the form of an interview, which is documented in a protocol.

6. The procedure for organizing a targeted reception.

6.1. The technical school considers applications for targeted admission received from state authorities or local governments, and makes a decision on the allocation of targeted places in the specialty, indicating their number within the target admission figures and within the quotas established by the founder - the Ministry of Health of the Chelyabinsk Region.

6.2. The number of places for targeted admission for each specialty is determined no later than a month before the start of accepting documents and should not exceed 15% of the total number of budget places for each specialty.

6.3. Applications for targeted places at the Technical College for full-time education are accepted until August 15, 2013.

6.4. If the parties contracting with the Technical School do not ensure competition among those sent to the target places, the admissions committee reduces the number of allocated target places, informs the relevant state or municipal authorities and those entering the target places about this.

6.5. Target places remaining vacant can be provided to persons participating in the general competition and are publicly available.

6.6. All procedures for targeted admission are documented in the protocols of the College’s admissions committee.

7. Enrollment in the Technical School.

7.1. The applicant provides the original state-issued education document by 12:00 p.m. 26.g. for enrollment in a budget and (or) paid form of education.

7.2. After the deadline for submitting original education documents, the director of the Technical School issues an order for the enrollment of persons recommended by the admissions committee for enrollment and who have submitted the originals of the relevant documents. An appendix to the enrollment order is a list of named persons. The order with an attachment is posted the next working day after publication at the information stand of the admissions committee and on the official website of the Technical School.

If the number of applicants exceeds the number of places, the financial support of which is provided by the budget of the Chelyabinsk region, the College carries out admission to study in specialties on the basis of entrance examinations in the form of a competition of state-issued documents on education based on their average score. The average score is calculated as the arithmetic mean of the grades in this document, rounded to the nearest hundredth of the average score of the educational document.

The passing score is determined by the average score of educational documents of all applicants for each specialty on a budgetary form of education as of 12:00 on 08/26/2013.

Persons entering each specialty are included in the lists of candidates for admission, taking into account the average score of the educational document in descending order.

Then the number of persons who passed the number of control figures for each specific specialty is determined. The passing score is the value at which an applicant passes to the available places in a specialty without competition with persons who have the same score.

For example: the number of places available for enrollment is 25. The line defining the first 20 places is 3.94 points. At the same time, there are 9 applicants who scored less than 3.94 points after number 20. The passing score will be determined as 3.94 points and persons who achieve this value will be enrolled. The remaining 9 people will compete for the remaining 5 places.

Their admission will be carried out by decision of the admissions committee, based on additional criteria - competition:

1. Persons permanently residing in the Chelyabinsk region - 15 points;

2. Children of medical workers - 20 points;

3. Persons with practical work experience in their chosen or related specialty - 20 points;

4. Persons living in rural areas - 10 points;

5. Persons who have completed the CPC in a medical specialty and have a certificate of the established form - 10 points;

6. Persons who have diplomas for success in studying certain subjects with a biological focus - 10 points;

7. Military personnel transferred to the reserve (within 3 years after dismissal) - 10 points;

8. Persons who have the highest grade in biology in their education document - 20 points;

10. Persons with sports ranks - 20 points;

11. Persons who have certificates of merit for success in sports competitions, amateur art competitions, and creative work competitions of any profile - 10 points.

Persons enrolled in the reserve who have less than 3.94 points (descending) are also identified, who can be enrolled in the corresponding specialty in the event of a number of applicants who passed the competition refusing to enroll or lack of timely confirmation of their consent (failure to submit on time. 00 o'clock 08/26/2016 original documents on education).

7.3. The date of completion of entrance examinations is considered to be the moment of announcement on the official website of the College and the stand of the selection committee of the list of persons by name, indicating the average score of the educational document, the enrollment of which is considered by the selection committee according to various conditions of the competition (hereinafter referred to as the list by name) - August 25, 2013.

7.4. After the deadline for submitting the original state-issued documents on education, the director of the Technical School issues an order on August 27, 2013 for the enrollment of persons recommended by the admissions committee for enrollment under various conditions of the competition and who have submitted the originals of the relevant documents. An appendix to the enrollment order is a list of named persons.

The order of admission, indicating the average score of the educational document, for budget places, is published on the information stand of the admissions committee and the official website of the Technical School on the day of their publication and must be available until December 31 of the current year inclusive.

7.5. Enrollment in the College, subject to availability of places, can be carried out until December 31, 2013.

8. Reception of foreign citizens.

8.1. Admission of foreign citizens to the College for training is carried out in accordance with these Rules and international treaties of the Russian Federation at the expense of the budget of the Chelyabinsk region, as well as under contracts with payment of tuition fees.

8.2. When submitting an application (in Russian) for admission to the College, foreign citizens, stateless persons, including compatriots living abroad, present the following documents:

A copy of the identity document of the applicant, or the identity document of a foreign citizen in the Russian Federation, in accordance with Article 10 of the Federal Law of 01.01.01. "On the legal status of foreign citizens in the Russian Federation";

The original of a state-issued document on education (or its duly certified photocopy), or the original document of a foreign state on levels of education and (or) qualifications, recognized in the Russian Federation at the level of a state-issued document on education (or its duly certified copy) , as well as in the case provided for by the legislation of the Russian Federation, a copy of the certificate of recognition of this document;

A duly certified translation into Russian of a foreign state’s document on education and (or qualifications) and its annex (if the latter is provided for by the legislation of the state in which such a document on education was issued);

Copies of documents or other evidence confirming that a compatriot living abroad belongs to the groups provided for in Article 4 of the Federal Law of 01.01.01 No. 99-F3 “On the state policy of the Russian Federation regarding compatriots abroad”;

6 photographs measuring 3x4cm (photos without headgear).

All translations into Russian must be made in the name and surname indicated in the identity document of a foreign citizen in the Russian Federation.
