ULTRASONIC METHOD FOR TREATING CHRONIC TONSILLITIS
The method includes: preparing the patient, preparing the device for operation (according to the corresponding section of the passport for the device), low-ultrasound sanitation of lacunae of the palatine tonsil through an intermediate ozone/NO-containing medicinal solution, contact low-ultrasound impregnation of ozone/NO-containing solutions of medicinal substances into the lacunae and tonsil crypts. After low-ultrasound sanitation of the patient’s oral cavity, in accordance with the parameters and regimens specified in paragraph 1 of section 4, local application or aerosol anesthesia of the pharynx is performed to suppress the pharyngeal reflex, with an anesthetic solution (lidocaine, pyromecaine, etc.) depending on the patient’s age and individual tolerance drug. Stage of low-ultrasound sanitation of tonsil lacunae through intermediate ozone/NO-containing saline(it is possible to use ozonated 0.1% hydrogen peroxide solution and 5-10% oil emulsion of the “oil in water” type) is carried out in an otorhinolaryngological or dental chair in a semi-sitting position of the patient, whose head is turned towards the sonified tonsil. After placing an applicator with a funnel of the required size on the tonsil, turn on the electric suction device, and set the applicator tap to the “open” position. Using the vacuum regulator on the electric suction device, the vacuum in the applicator system is set to about 0.2-0.3 kgf/cm2, which is necessary for reliable fixation of the applicator on the tonsil and additional vacuum drainage of lacunae from pathological contents, as well as suction and circulation (through a funnel) of the medicinal solution. Used as an intermediate medicinal medium for sonication of the tonsils: ozonated 0.1% hydrogen peroxide solution, saline solution, 5-10% oil-in-water emulsion and other antiseptics of the oxidizing group, in combination with NPUS, have a pronounced bactericidal, fungicidal and virucidal effects on the source of infection. After filling the applicator funnel with an antiseptic solution, the sonication exposure is set, the control unit is turned on, and the tonsil is treated with ultrasound through a circulating solution of ozonized antiseptic using a waveguide-tool “VI3” under the parameters and modes specified in paragraph 2 of section 4, i.e. in the mode of developed cavitation. The exposure time for one amygdala is 60-90 sec. Taken together, occurring in a liquid medicinal environment under the influence of high-amplitude low-ultrasonic ultrasound: cavitation, variable sound pressure, acoustic currents and high-speed liquid jets directed to the surface of the tonsil, ensure the destruction and washing away of surface protein layers, expansion and opening of the mouths of lacunae and crypts and leaching from their pathological contents, including caseous plugs, initiation of “direct” and “reverse” ultrasonic capillary effects in the parenchyma and capillary-porous lacunar system of the tonsil, accompanied by extraction of pathological contents and drainage of lacunae and crypts, impregnation of ozone/NO-containing medicinal solution in them. This ultimately leads to the intensification of the processes of clearing the tonsil from pathological contents, inactivation pathogenic microflora, deposition of the medicinal substance into the cleared, during the process of sanitation, lacunae and crypts of the tonsil, exerting a positive effect on it therapeutic effect. The ozone/NO-containing medicinal solution circulating in the funnel near the tonsil (during the entire sonification session), on the one hand, cools the radiating end of the waveguide-instrument “VI3”, preventing thermal damage to the tonsil, and on the other hand, removes the “spent” portions of the solution pathological contents into the collection jar of the electric suction device. After voicing the tonsil, the applicator tap is set to the “closed” position, the control unit and the electric suction device are turned off, and the patient is asked to hold his breath. At this moment, the applicator is removed from the tonsil and the patient assumes a sitting position. The second tonsil is treated in the same way.
The next step is contact ultrasonic impregnation of a medicinal solution that stimulates the reparative regeneration of the tonsil parenchyma (lysozyme, human leukocyte interferon, hydrokartisone suspension, honey-propolis mixture, ozonated 5-10% oil emulsion of the “oil in water”, etc.). For this purpose, a “VI9” waveguide instrument with a bowl-shaped radiating end is used, which is connected to the acoustic system before the procedure. A technological gasket (2-3 layers of gauze) soaked in a medicinal solution is fixed to the cup-shaped emitting end, after which the waveguide-instrument is intermittently (without force) pressed against the os surface of the tonsil. The control unit is turned on and, in the acoustic flow mode, low-ultrasonic impregnation of the medicinal solution is carried out under the parameters and modes specified in paragraph 3 of section 4. The exposure time of sonification of one tonsil is 20-30 seconds. Treatment of patients with chronic tonsillitis is usually carried out on an outpatient basis. Before undergoing NUS therapy, patients must undergo a general clinical examination. During the examination, the otolaryngologist should pay attention to Special attention on the condition of the nose, pharynx and oral cavity. If adenoids are detected, they must be removed or treated using NUS according to the method described below. Since the above ozone/NO-containing substances are used as antiseptics medicinal solutions, as well as solutions of the oxidizing group, then, along with the effect of LFUS, pronounced tissue oxygenation is observed, which is an essential point in the treatment of chronic tonsillitis. Starting from the 3-4 treatment session, patients, in addition to NChUS sanitation of the tonsils through an antiseptic solution, additionally undergo the stage of contact NChUS impregnation of medicinal substances into the lacunae, crypts and parenchyma of the tonsil with the creation of a long-term depot of the drug. At the last treatment sessions, it is possible to carry out only low-ultrasonic impregnation of medicinal substances into the lacunae, crypts and parenchyma of the tonsil. At the end of the course of treatment, the patient is examined by an otolaryngologist and all changes identified during an objective examination are recorded in outpatient card. To study the changes occurring in the tonsils under the influence of low-frequency ultrasound, it is advisable to perform pharyngomicroscopy. In this case, you should pay attention to the condition of the mucous membrane in the area of the mouths of lacunae, palatine arches, consistency lymphoid tissue and the severity of the scarring process. It is advisable to carry out a follow-up examination of patients after treatment with NCUS after 1, 3, 6, 12 months, 1, 5, 2 and 3 years. Sometimes, in some patients, after 2-3 procedures of NCUS sanitation of the tonsil (during sanitation of lacunae and crypts), there may be moderate symptoms exacerbation of a chronic process, which are subjectively manifested by a slight sore throat, sore throat and low-grade fever. Pharyngomicroscopy may reveal slight hyperemia and swelling of the mucous membrane of the tonsils and palatine arches. General state the patient does not suffer. In this case, it is necessary to take a break from treatment for 2-3 days, calcium supplements and antihistamines. After the reactive phenomena from the tonsils subside, the course of treatment must be continued. Note - in the presence or occurrence of acute respiratory disease, it is possible to carry out NCUS therapy for a patient with chronic tonsillitis only 10-15 days after its cessation.
